1. AGREEMENT TO OUR TERMS

These Terms and Conditions ("Terms") constitute a legally binding agreement made between you, whether personally or on behalf of an entity ("you"), and Yerli Medical PLLC ("we," "us," or "our"), concerning your access to and use of our website at yerlimd.com and any related media, content, and services (collectively, the "Services"). By accessing or using the Services, you agree that you have read, understood, and agree to be bound by all of these Terms. If you do not agree with all of these Terms, you are expressly prohibited from using the Services and must discontinue use immediately.

2. OUR SERVICES

Yerli Medical PLLC is a New York-based adult primary care practice operated by a single provider, Dr. Behiye Almila Yerli, a physician licensed in internal medicine in New York State. This website provides general information about our practice and allows you to learn about our services, contact us, and schedule appointments. All patient care is personally provided by Dr. Yerli. We do not connect patients with multiple physicians, third-party providers, contractors, or out-of-state clinicians.

3. NO MEDICAL ADVICE; NO DOCTOR-PATIENT RELATIONSHIP THROUGH THE WEBSITE

The content on this website is provided for general informational purposes only and is not intended to be a substitute for professional medical advice, diagnosis, or treatment. Your use of the website does not create a physician-patient relationship. A physician-patient relationship is established only through a scheduled consultation and an in-person or telehealth evaluation, as clinically appropriate. Always seek the advice of your physician or other qualified health provider with any questions you may have regarding a medical condition. Never disregard professional medical advice or delay seeking it because of something you have read on this website.

4. MEDICAL EMERGENCIES

The Services are not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use this website or any messaging or appointment-request feature to seek emergency care.

5. TELEHEALTH AND PATIENT LOCATION

Telehealth (virtual care) visits are offered only when clinically appropriate and only when you are physically located in New York State at the time of the visit, where Dr. Yerli is licensed to practice medicine. If you are outside New York State at the time of a requested virtual visit, the visit will not proceed, and you will be advised to schedule care when physically located in New York or to seek care from a licensed clinician in your current state. 6. APPOINTMENTS AND THIRD-PARTY SCHEDULING

Appointment scheduling may be facilitated through third-party platforms, including Zocdoc. When you use a third-party scheduling service, you are also subject to that provider's terms of use and privacy policy. We are not responsible for the practices, content, or availability of third-party platforms. Appointment requests are not confirmed until you receive confirmation from our practice. We reserve the right to reschedule or decline appointments as necessary.

7. NO MEDICATION SALES OR PHARMACY SERVICES

We do not dispense medications, sell medications online, operate as a pharmacy, or partner with any pharmacy for medication sales or fulfillment. Any prescriptions are issued only when clinically appropriate as part of a legitimate physician-patient relationship and are fulfilled through your chosen licensed pharmacy.

8. ELIGIBILITY

The Services are intended for users who are at least 18 years of age. By using the Services, you represent that you are at least 18 years old or the parent or legal guardian acting on behalf of a person for whom you are authorized to seek care. The Services are not directed to children under 18, and we do not knowingly collect information from them.

9. INTELLECTUAL PROPERTY RIGHTS

Unless otherwise indicated, the Services and all source code, databases, functionality, software, website designs, audio, video, text, photographs, logos, and graphics on the Services (the "Content") and the trademarks, service marks, and logos contained therein (the "Marks") are owned or controlled by us or licensed to us, and are protected by copyright, trademark, and other intellectual property laws. The Content and Marks are provided on the Services "AS IS" for your personal, non-commercial use only. 12. THIRD-PARTY WEBSITES AND CONTENT

The Services may contain links to other websites ("Third-Party Websites") as well as content belonging to or originating from third parties.
